2. Complete the form, and obtain the appropriate signatures.
3. Fax, mail, scan or e-mail the form to our office:
4. A certified case manager from Premier Senior Services will contact you to set up a personal and customized initial assessment.
5. You’ll receive a summary of recommendations, and we’ll discuss your options.
6. Once a course of action has been decided on, you’ll begin to receive a monthly bill for our services.
7. Yourcertified case manager will arrange the agreed-upon services (including NC elder care case management, NC geriatric case management and North Carolina elder care management), and check in regularly to make sure the senior is getting everything he or she needs.
