The nurse case manager met with my mother at the hospital and assisted in communication between the hospital staff and the family. Prior to discharge from the hospital the case manager communicated and facilitated a smooth transfer from hospital to rehab and back to her assisted-living facility. A home assessment was completed and special needs and safety issues were addressed. All medical equipment and supplies were obtained and delivered directly to my mother’s door. Ongoing follow up has continued to address any potential health care concerns and needs. My mother’s care manager has always been accessible and eager to assist us in any way possible.

Dealing with MS and her family has been a rewarding experience for me, as I assisted in providing eldercare case management to MS following her recent shoulder and hip fracture. I feel that my services minimized the stress and frustration the family experienced as they tried to understand medical terminology and select the best treatment facilities available for their mother. The family and MS both wanted her to eventually return to the assisted living facility rather than consider nursing-home placement. After installing an emergency call system and purchasing a few pieces of durable medical equipment, MS can safely maneuver in her apartment. Communication between the assisted-living facility’s nursing staff and this case manager has allowed the case manager to quickly identify any changes in MS’s medical condition and modify the care plan and recommendations accordingly.
